4) Improvements in efficiency and effectiveness of patient care

5) Improvements in quality of care

6) Reduction in costs of care

7) Improvements in outcomes of care

8) Population health management using programs such as Accountable Care Organizations (ACO’s) and Clinically integrated networks (CIN’s)

9) Wearable health devices such as fitness trackers or blood pressure and blood sugar monitoring devices. And how the data from these devices is going to be integrated with electronic health records within the providers offices.
