Y​‌‍‌‌‍‌‍‍‌‌‌‍‌‍‍‌‍‍‍​ou, charge nurse poole will conduct an interview with Alicia Balewa director of safe headspace( a non-profit that works on improving outcomes for TBI ( Traumatic brain injury) patients). The patient is Mr. Nowak. During his cholesterol screening he was having trouble with his balance. He believes it could be related to his hypertention and one time he was hospitalized for a traumatic Fall from a tree many years ago. He expressed distress over this and believes this might be the beginning of a rapid decline. This interview will include a public health improvement initiative that might apply to Mr. Nowak's care. You believe he will benefit from Balewa's TBI and PTSD program. Questions and responses to include: What populations did your public health improvement initiative study? Which treatments showed the strongest improvement? Have you tried anything that has not work? Mr. Nowak has moderate TBI and has suffered almost 40 yrs, and no history of PTSD. Have you separated your population abs studied each separately? 1) Evaluate the expected outcomes of the population health improvement initiative that were not achiev​‌‍‌‌‍‌‍‍‌‌‌‍‌‍‍‌‍‍‍​ed and described the outcomes that were achieved. Describe their positive effects on the communities health and any variance across demographic groups. Identify the factors(community, environmental, resources, communication, etc) that may have contributed to any achievement shift falls. 2)Propose a strategy for improving the outcomes of the population health improvement initiative. Ensure that all outcomes are being addressed on the best available evidence. Describe the corrective measures you would take to address the factors and illustrate improved outcomes if your proposed strategy is enacted. Design interventions to improve health. 3) Develop an approach to personalize patient care that incorporates lessons learned from the population health improvement initiative outcomes. Explain how the outcomes and lessons learned informed the decisions you made in your approach for personalizing care for the patient with a health condition related to the population health concern addressed in the improvement initiative.
