Here the patient does not have allies and is forced to rely on the senses to aid the understanding.

The third awareness context according to the article is the overt suspicion. In this case, the patients suspicion about the impending death increases although the patient chooses to keep silent about it for some time. More often than not, the patient can be able to keep the suspicion long enough but at last, the true status will be confirmed. What happens during this period is that the staff also suspects and wonders whether the patients are indeed expecting the worst or not. In most occasions, even when the hospital personnel suspects that the patient expects the worst, the personnel will keep it a secret until the patient reveals it.
