Ethical Issues in Surgery and Patient Resuscitation

Part I

As a summary, the case involves an 89-year old male who was presented to the Emergency Department with a self-inflicted gunshot wound. The ethical issues arising in this include the surgeon coerced to perform the surgery despite an expected poor outcome. Secondly, the patient signed Do Not Resuscitate form when he urgently requires life-saving interventions raises ethical issues. There are three approaches when considering ethical dilemma in health set up. First is consequentialism which is also called utiliarism. This approach analyzes the consequences of a particular cause of action. All the parts of a particular decision are given equal weight before undertaking any action. Second approach is deontology which focuses on what is morally right in a particular situation. Deontology doesnt consider consequences but bases actions on the duties and morality of an individual. Last approach is virtues. This considers ones knowledge and makes actions depending on the ethical constitution and professionalism of an individual. (Herring, 2014)

The first ethical dilemma in this situation is whether the surgeon should give in to relatives demand and perform a surgery which he knows very well has a poor prognosis or decline to operate on the patient. Although the surgeon understands the outcome, he or she lacks absolute evidence and proof that the surgery wont save the patient. The neurosurgeon should give in to the client demands and proceed to perform the surgery. This may lead to unnecessary expenses. Legally, the surgeon has done everything in his means to save the patient even though the patient may not survive. Also, the relatives could have sued the surgeon for declining to perform a life-saving procedure. Consequently, the doctor will preserve his job and also satisfy the client.
