The development of ICD-10 coding standard for health records in 1994. Then the computer era of the 2000s welcomed a statistic by the Institute of Medicine that most people lost lives to errors in the records. This drove president George W. Bush to call for Electronic Health Records. It was picked up by Obama in 2009 where he signed the Health Information Technology for Economic and Clinical Health (HITECH) Act
